Priority Violations: Employee did not wash hands when changing tasks that involved working with open food. Foods being reheated in the hot holding unit. Hummus at 43 degrees in the prep table.

Priority Foundations Violations: No paper towels at the back hand-washing sink in the ware washing area. Back dispenser is broken and there is no front dispenser. Not all food items are date marked as needed. Facility does not have a thermometer to check cold food temperatures. Label holder is made of unfinished wood. Item is stored in the food preparation area.

Core Violations: No certified food protection manager. Employee working with open food is not wearing an effective hair restraint. Shelves and supports in the back storage area are unfinished wood.

Priority Violations: Food in the single door cooler is not at 41 degrees or below.

Core Violations: Some food employees working with open food are wearing visors which are not effectively restraining their hair. Legs of the cutting/boxing table are visibly dirty. Handle and door of the walk-in cooler is visibly dirty. Women's restroom door is no longer self closing.

Core Violations: No certified food protection manager. Fresh water tank must be large enough to supply water for the entire day and processes.

Core Violations: The three-compartment sink and handwashing plumbing was observed to not be in good repair, the three-compartment faucet cannot be turned off independently.

Priority Violations: Mechanical dish machine labeled that it is using hot water to sanitize reaching 143.9 degrees measured by a dish simulating irreversible registering temperature indicator. Mechanical dish machine is hooked up to chlorine sanitizer but 0 ppm chlorine sanitizer detected in mechanical dish machine.

Priority Foundations Violations: No hand cleanser available at bar handwashing sink. Rinse temperature of mechanical dish machine labeled that it is sanitizing with hot water reading 148 degrees on mechanical dish machine thermometer.

Core Violations: Boxes of single-service to-go containers and cutlery stored on floor in dry storage area. Toilet room opening into kitchen area lacks self-closing door.

Tokyo House Of Rogers

401 N. 46th St., Rogers

Priority Violations: Beef at 46 degrees and chicken at 45 in cold hold cases across from grill.

Priority Foundations Violations: In almost every cold case and walk-in raw meat was stored above chopped veggies, and other ready to eat food. Raw meat was also placed behind raw ready to eat vegetables and carried over them when prepping. Food on the floor of walk-in freezer, fridge and dry storage. Scoop without handle used in rice bin by walk-ins.

Core Violations: No covered receptacle for feminine hygiene products in employee bathroom. Missing and damaged tiles in floor of mop sink.

Priority Violations: Sanitizer concentration for the chlorine spray bottle in the kitchen used for food contact surfaces is too strong.

Priority Violations: Facility food preparation sink observed with water inlet spray nozzle hanging inside sink lacking air gap between water inlet and flood level rim of sink.

Priority Foundations Violations: Pasta salad and potato salad offered for grab and go lacks labeling information.

Priority Violations: Tempura shrimp being held at 58 degrees and spicy crab mixture being held at 51 degrees on cold holding preparation table.

Priority Violations: Tempura batter by tortilla grill was sitting in an off cold hold unit.

Priority Foundations Violations: Packaged food has ice buildup in walk-in freezer.

Smokin' Joe's Ribhouse 3

2504 E. Central Ave., Bentonville

Priority Violations: Diced ham in salad prep table was 44 degrees.

Priority Foundations Violations: None.

Core Violations: Walk-in refrigerator built-in thermometer is broken. Screening for outdoor cooking area has places that do have screens in place. The screens do not go all the way to the ground and there are gaps where the old shed and the new part are attached that need to be screened or filled in. Small bowls are being used as a scoop for jalapenos in salad prep table and the bulk container of seasoned salt. Water is standing in bottom of sandwich prep table cooler in kitchen. Door seal on walk-in freezer is torn.

Speedy's #1200

Priority Violations: Open container of pizza sauce, open packages of hot dogs and various pizza toppings are not date marked as needed. None of the food in the hot case is marked with the time the items need to be used by.

Priority Foundations Violations: Facility does not have a written procedure per the regulations to use time as a safety control for the foods displayed in the hot holding case. Door handles on the prep table, and upright food coolers are visibly dirty. Interior and doors of the prep table and upright coolers are visibly dirty. Floor and side of fryer have an accumulation of food and grease. Floor in the kitchen is visibly dirty especially around and under equipment. Both sides of the swinging door between the kitchen and customer area are visibly dirty.

Core Violations: One set of shelves in the walk-in cooler is rusty in places. The rack in the hot food display case where the chicken is directly in contact with the rack has what appears to be built up grease and food debris, especially at the connecting joints. Arkansas Retail Food Permit is not posted in customer view.

Multiple unlabeled shakers and squeeze bottles throughout prep and sushi area. Cardboard boxes of food items being stored directly on floor of walk-in freezer. Multiple employees wearing wrist watches. Top layer of concrete is wearing away in the dish area.

Priority Violations: Employees did not wash hands between task changes or when coming back into the kitchen. Dirty dishes in the hand washing sink. Tofu stored above the load line. Product temperature is 54 degrees. Container of bean sprouts stored on a block of ice. Garlic that was roasted in oil is being stored at room temperature. Package of raw meat stored in a container in water on the floor under the mop sink in an area where water was standing on the floor.
